The information which are required to file a charge for discrimination are as follows:
(a)The complainant's name, address, and phone number;
(b)The name, address, and phone number of the respondent manager, business office, or union that
is claimed to have discriminated and number of workers (or union individuals), if known;
(c)A brief of the alleged discrimination (the occasion that made the complainant trust that his or
her rights were abused); and
(d)The date(s) of the charged violation(s).
The above procedure is however different for the federal employee.
